The INSPIRE Questionnaires are qualified as a Medical Device Development Tool (MDDT) by the FDA (Q191073).

USER AGREEMENT: INSPIRE captures perceptions and experiences with AID thus it is appropriate for use in people with diabetes either on AID or not using AID. It is free to use and by downloading a form below, you agree to 1) not alter the form and administer as is, and 2) acknowledge the use of INSPIRE in publications.

We recommend the following scoring and interpretation:

1.     Ensure that the scoring template used (paper or electronic) ranges from Strongly Agree to Strongly Disagree. The five response options should be scored as 4 = Strongly Agree, 3 = Agree, 2 = Neither Agree or Disagree, 1 = Disagree, and 0 = Strongly Disagree. All items are responded to on this same scale and there are no reverse-scored items. If N/A is checked do not include that item in scoring.

2.     Calculate a mean score for the scored items; this will range from 0 to 4.

3.     Multiply the mean score calculated in step two by 25. This score represents a scaled score from 0 to 100 and was done in order to provide similar scaling as other commonly used quality of life scales. This final score will range from 0 to 100. Higher scores indicate more positive appraisal of automated insulin dosing.

INSPIRE Questionnaire For Adults For adults 18 years and older, this survey should be used.
INSPIRE Questionnaire For Parents For parents or caregivers of people with diabetes, this survey should be used. It is assumed the respondent is an adult.
INSPIRE Questionnaire For Partners For partners of adults with diabetes, this survey should be used. It is assumed the respondent is an adult.
INSPIRE Questionnaire For Youth For youth ages 8 years and older, but not older than 18, this survey should be used.
